Employee Elective Benefits

If you or your family experience an injury or illness, you shouldn’t have to worry about how you are going to pay your bills. Voluntary benefits can protect your finances during the most challenging times.

Eligibility for benefits is defined as Full-Time (FT), Regular-Time (RT) and Part-Time (PT).

Key Dates

Initial Enrollment

Initial enrollment is the 30 calendar days you have, beginning on your date of hire, to enroll yourself and your dependents in benefits. If you don’t enroll within 30 days of becoming eligible, you’ll be required to wait until the next annual Open Enrollment to elect benefits.

Ongoing Enrollment

Select products are available year-round to Hy-Vee employees, including Life Insurance and Disability Insurance. A dedicated Hy-Vee Elective Benefits Advisor can help you choose the coverage that is right for you.

Annual Open Enrollment Period

Annual Open Enrollment for Hy-Vee employees typically occurs in November each year. You can make changes to your benefit elections, including enroll, waive benefits, and add or remove dependents.

Types of Coverage

Eligibility for each is noted as Full-Time, Regular-Time and Part-Time. Unless otherwise listed, benefits may only be elected during initial enrollment as a new hire, within 30 days of a qualified life event or during your designated Annual Open Enrollment period.

The below list is an overview of coverage. The terms and conditions of coverage are set forth in the insurance policies. Exclusions and limitations apply.

Life Insurance (FT, RT & PT)

As a Hy-Vee employee, you have access to an exclusive life insurance offer through EMC National Life. Employees are eligible for up to $100,000 in life insurance without having to answer any health questions. We offer whole and term life options. Rates are locked, and plans will stay with you no matter where your career takes you.

Long-Term Disability (FT, RT & PT)

Protect your paycheck with long-term disability coverage following a 13-week short-term disability benefit if you’re still unable to return to work. All premiums will be returned to you (less any claims) at the end of the contract if the optional return of premium rider is elected. Qualify for up to 70% of your income in monthly benefits without a physical. Rates are locked, and plans will stay with you no matter where your career takes you.

Hy-Vee Contributory Benefits – Part-Time Employees (PT)

Hy-Vee part-time employees who have been part of the Hy-Vee family for at least six months and average 20 hours or more per week are eligible to have Hy-Vee help pay for part of their health and wellness benefits, including basic life insurance, short-term disability, limited medical insurance and dental insurance, which can help substantially lower your benefits costs.

Virtual Health Connect – Part-Time Employees (PT)

Hy-Vee employees who want to supplement existing health insurance coverage, Virtual HealthConnect may be a great option for you and your family! Through Virtual HealthConnect, Hy-Vee has bundled coverage for: Telehealth services with no copay, hospital confinement benefits, emergency room sickness benefits, term life insurance, and prescription drug discounts. This benefit is available to those who are contribution eligible.

Limited Medical Insurance (PT)

Hy-Vee’s Limited Medical plans provide protection against basic medical expenses. These plans provide first-dollar coverage for medical expenses, such as hospitalization, surgeries, doctor visits, ER sickness visits, lab and blood work, X-rays, MRIs and prescriptions.

Telehealth benefit: MDLIVE gives you access 24 hours, seven days a week to a U.S. board-certified doctor through the convenience of phone, video or mobile app visits. SupportLinc Employee Assistance Program offers unlimited telephonic access to behavioral health professionals to help individuals with a variety of life and mental health issues, as well as three in-person counselor visits.

Limited Medical insurance is NOT major medical insurance and is not intended to replace major medical insurance.

Short-Term Disability (PT)

Short-term disability pays you up to 60% of your weekly earnings if an off-the-job illness or injury prevents you from working. After a seven-day elimination period, you’ll receive benefits for up to 13 weeks.

Dental (FT, RT & PT)

Under dental insurance, preventative care, such as cleanings, exams and X-rays are covered at 100%. Enrollees have access to the largest network of dentists in Iowa and nationwide. That means you can potentially save even more when you see an in-network dentist. Coverage is also available for restorative services, major services and child orthodontia.

Vision (FT, RT & PT)

For eye exams and standard lenses, you’ll only pay affordable copays. For expenses like frames, follow-up exams, optional lens coatings, and elective Lasik and PRK vision correction surgeries, you’ll receive allowances and percentage discounts to help reduce your out-of-pocket expenses. The plan is flexible and gives you access to a nationwide network of eye care providers.

Accident (FT, RT & PT)

Accident insurance provides benefits to help cover out-of-pocket medical expenses related to an accidental injury. Benefits are payable directly to you for expenses such as emergency room or doctor’s office visits, X-rays, ambulance, hospital confinement, dislocation and fractures, and more. Benefit amounts vary by claim and plan type.

Critical Illness (FT, RT & PT)

Critical illness coverage works in two ways. First, you receive cash benefits when diagnosis of a covered critical illness occurs. Second, it encourages you to stay healthy before a critical illness strikes by paying insureds a benefit for receiving a covered wellness screening once annually for each person on your plan. Dependents diagnosed with a covered critical illness will receive 50% of the benefit amount chosen by the employee.

Covered illnesses include:

  • Advanced Parkinson’s disease
  • Stroke
  • Benign brain tumor
  • Invasive cancer
  • Complete blindness
  • Coronary artery bypass surgery
  • Complete hearing loss
  • End stage renal failure
  • Heart attack
  • Paralysis
  • Carcinoma in situ
  • Advanced Alzheimer’s disease

Cancer (FT & RT)

Cancer insurance provides fixed benefits for early detection and treatment of certain types of cancer. It also includes benefits for other related expenses, including drugs and medicine, new or experimental treatment, hair pieces, hospital confinement, chemo, radiation and surgery. Benefits are payable for one wellness screening annually for each person on your plan. Benefits are paid directly to you.

Hospital Indemnity (FT, RT & PT)

Hospital indemnity coverage pays you directly when you or a covered family member has a hospital confinement. Benefits are payable directly to you and can be used as you see fit to help cover hospital bills, lost income, childcare or any other expenses.

Identity Protection (FT, RT & PT)

Hy-Vee recognizes the exponential increase in fraud and scams as your digital footprint expands, and the vulnerabilities that can result from having sensitive personal information exposed. That’s why Hy-Vee offers identity protection benefits through IdentityForce. Their team of experts is here to provide Hy-Vee employees with world-class identity theft protection plans built to proactively monitor your information. Should your identity become compromised, IdentityForce will work to restore your identity to pre-theft status.

Health Savings Account (HSA) (FT & RT)

With select health insurance plans you have the option to set up a Health Savings Account (HSA). HSAs let you set aside money on a pre-tax basis to pay for qualified medical expenses.
