Group Universal Life Insurance Claim

To begin your claim, contact Midwest Heritage at 515-343-5047. A customer service representative will ask you for the following information to get the claim form started, and then mail the forms to you for completion.

Death Claim:

• Employer, a designated family member or representative notifies Midwest Heritage of insured’s death. Midwest Heritage will send a claim form to the designated beneficiary upon notification.

• Beneficiary must provide certified copy of death certificate with completed claim form

Group Universal Life Insurance regarding a long-term care claim:

• Employer, a designated family member or representative notifies Midwest Heritage of insured’s long-term care claim.

• Midwest Heritage will send a claim form to the designated family member or representative. Request an itemized billing statement for long-term care from the provider showing date(s) of service, services provided, and how much you were charged for each service (For actual claim, keep all receipts associated with the claim and submit copies with the claim form.)

Send complete the claim form with the appropriate itemized bills and death certificate to Midwest Heritage Insurance, 1025 Braden Ave., P.O. Box 737, Chariton, Iowa 50049.
