Critical Illness Wellness

Allstate makes it easy to file your wellness claims.

1. Log into the Allstate Benefits website at: www.allstateatwork.com/mybenefits and file an “Express Wellness Claim.” This option is the quickest way to have your claim processed. In order to file an “Express Wellness Claim” you will need the date of service, test performed, charges for the test, and the information of the facility that performed the test. Choose direct deposit and have your claim payment deposited into your bank account in as little as three days! Registration is required.

2. Complete a paper Wellness Claim Form and submit it to American Heritage Life Insurance Company by mail or fax along with a copy of the documentation for the Wellness test that was performed. To obtain a claim form call 515-343-5047, or email us at CSR@mhbankins.com. Paper claim forms submitted can take up to 10 business days for processing. Wellness claims under the cancer or critical illness benefits can be faxed directly to 866-424-8482.

3. See a Midwest Heritage Representative during a store visit.

According to the plan guidelines, a wellness benefit will be paid only one-time per year for each covered family member. Listed below are the common wellness screenings that can be considered for a wellness claim. (Full-time employees on Hy-Vee’s core medical plan can submit their biometric screening, which includes a lipid panel.)

Biopsy for skin cancer
Blood test for triglycerides
Bone Marrow testing
Pap smear, including ThinPrep Pap Test
EKG (Electrocardiogram)
Chest X-Ray
Stress test on bike or treadmill
Thermography
HPV Vaccination (human papillomavirus)
CEA (Carcinoembryonic antigen- blood test for colon cancer)
Ultrasound screening of the abdominal aorta  or abdominal aortic aneurysms
Lipid panel (total cholesterol count)
Mammography, including breast ultrasound
Flexible Sigmoidoscopy
Hemocult stool analysis
Serum Protein electrophoresis (test for myeloma)
Colonoscopy
Doppler Screening for Carotids
Doppler Screening for peripheral vascular disease
Echocardiogram
CA-15 (cancer antigen 15-3 blood test for ovarian cancer)
PSA (prostate specific antigen- blood test for prostate cancer)
